Comments:

An unannounced monitoring inspection was initiated on 12/10/19 by 2 licensing inspectors and completed on 12/13/19 by 1 licensing inspector. There are currently 12 residents in care at the facility. Previous violations were reviewed and corrected. During the inspection, a tour of the facility was conducted, a sample of 6 resident and 3 staff records were reviewed, facility documentation was reviewed, required postings were reviewed, first aid kit and emergency food/water supplies were checked, medication pass was observed, medication administration records and physician orders were reviewed, and staff and resident interviews were conducted. An exit meeting was held with the administrator. All new personnel records were reviewed since the last inspection for criminal history record reports and all were in compliance.
