Comments:

The Piedmont Licensing Office received an anonymous complaint in regards to a resident at Bentley Commons. The complaint alleges that a resident was found outside of the facility in cold weather during night time hours. The LI for Bentley Commons conducted an unannounced complaint investigation at the facility on 3/2/2020 from 9:30am until 11:30am. A review of a resident record was conducted as well as review of other facility documentation. A tour of the physical plant including the front outside grounds of the facility was conducted and interviews were held with staff. Based on all information available for review it was noted that a resident was observed on the morning of 9/25/2019 to be sitting in a wheelchair on his porch that was accessible from a door in his room that leads to the outside.
