Comments:
Two licensing inspectors conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 01/22/2020. The inspection started at 10:20 am and concluded at 11:24 am. Interviews were conducted with one resident and staff members. Observations were made regarding resident care. The preponderance of evidence found during the inspection does not support the allegations made within the complaint; therefore, the complaint is not valid. An exit meeting was conducted with the administrator and other key staff on 01/24/2020. A follow up phone call was made on 01/27/2020 to a staff member not present during the inspection. As a result of this inspection one violation is being cited. Please complete the columns for "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 calendar days (02/08/2020 ) of receipt. Description: Based on observations made during the tour of the building, the facility failed to keep the medication storage area locked.

EVIDENCE:
1. Two licensing inspectors observed the medication cart in the assisted living facility hallway to be unlocked and unattended. The licensing inspectors checked each drawer of the cart and all of them were unlocked.
2. The medication cart remained unlocked and unattended for approximately three minutes until the in-charge medication tech could be found.
3. Staff # 3 who was the medication tech in charge was found in a resident's room administering medications. She stated she was not aware the medication cart was unlocked when the licensing inspector brought it to her attention. She immediately went and locked the cart.
4. This facility serves a mixed population meaning there are residents within the facility that have serious cognitive impairments and cannot recognize danger.

Plan of Correction: The Med tech locked the cart immediately upon notification. The Director of Nursing (DON)/designee educated the Med Tech on maintaining the med cart to be locked at all times when not in use and/or not attending. The Director of Nursing (DON)/designee notified OmniCare and requested service on Med Cart to rule out any mechanical issues. OmniCare technician serviced cart and identified a lid that was jamming the drawer which prevented it from locking accurately. The lid was removed immediately and the drawers were serviced with new slide rails/guards installed. ALF Nursing Staff will be educated regarding maintaining the med cart to be locked at all times when not in use.or not attending by 02/05/2020. 01/29/2020 by the DON/designee. The re-education will include a posttest to validate understanding. New Hires nurses, including agency nursing, will be provided education and postttests during orientation by the DON/designee. The DON/Designee will monitor the ALF unit to ensure the maintaining of the med cart being locked at all times when not in use and/or not being attended randomly 2 times daily for 4 weeks; one time per week for two weeks with corrective action upon discovery.
Trends identified will be reported by DON/designee monthly to the Quality Improvement Committee (QIC) for any additional follow up and/or in-servicing until the issue is resolved and randomly thereafter as determined by the QIC committee. [sic]
