This inspection was conducted by Licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 10/16/2020 and concluded on 11/23/2020. A self reported incident was received by the Department regarding allegations in the area of standard 150 B. The evidence gathered during the investigation supported the allegation of non compliance with the standard or law and violation was issued. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return to the Inspector within or by 10 days. You will need to specify how the deficient practice will be corrected. Just writing the word ?corrected? is not acceptable. Your plan must contain 1) steps to correct the non-compliance with the standards, 2) measures to prevent the non-compliance from occurring again, and 3) person(s) responsible for implementing each step and or monitoring any preventative measure(s).

Standard #: 22VAC40-73-150-B Description: Based on correspondence with the facility, the facility failed to immediately appoint a qualified acting administrator so that no lapse in administrator coverage occurs.
Evidence: The previous Administrator reported to the Division of Licensing Programs that her last day of employment was October 16, 2020. On October 21, 2020 a letter was submitted by the facility and signed by a Managing Partner stating that the facility had "secured a new executive director" who will "commence on site full time as administrator no later than 11/20/2020." The letter stated that the Managing Partner would "continue to remain on site..." The Managing Partner was asked via email if the Department of Health Professions (DHP) stated that he was qualified to serve as the Acting Administrator by education until your permanent administrator arrives on-site. An email response was received on October 30, 2020 and stated, "The DPH responded by requiring me to go through the process of filing an Acting Administrator or Application, which is identical to the ALF Administration AIT process." The facility did not have an Administrator or Acting Administrator of record for a period of five weeks beginning October 16, 2020 and ending November 20, 2020 who was qualified through the Department of Health Professions.Plan of Correction: The facility has secured a Full time Licensed Assisted Living Administrator, start date 11/20/2020. The facility will ensure there is always a Licensed Assisted Living Administrator or Qualified Acting Administrator on premises Fulltime as required by Licensing standard.
