Type of inspection: Other Date of inspection and time the licensing inspector was on-site at the facility for the day of the inspection: 10/18/22 1 pm -4pm The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents present at the facility at the beginning of the inspection: 19 in safe, secure environment The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility with the facility?s exterminator, representatives from the City of Richmond (health department, DSS, building compliance) and a representative from the AL small home providers. Number of resident records reviewed: 0 Number of staff records reviewed: 0 Number of interviews conducted with residents: 2 Number of interviews conducted with staff: 3 Observations by licensing inspector: resident rooms/bedrooms, common areas Additional Comments/Discussion: A non-mandated monitoring inspection was conducted to follow-up on compliance with the (POC) submitted. The facility was in compliance with the Plan of Correction submitted on 10/9/22. Situation will continue to be monitored by the local health department and VDSS. An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
