1020- THE FACILITY HAS A MIXED POPULATION -ON THE 3rd FLOOR - At least two direct care staff members shall be awake and on duty at all times in the building. 530.- Any resident who does not have a serious cognitive impairment shall be allowed to freely leave the facility. 1040-Residents with serious cognitive impairments may be limited but not prohibited from exiting the facility or any part thereof. Before limiting any resident from freely leaving the facility, the resident's record shall reflect the behavioral observations or other bases for determining that the resident has a serious cognitive observations or other basis for determining that the resident has a serious cognitive

Type of inspection: Complaint Date of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/26/23 11am -12 :30 p The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on 6/13/23 regarding allegations in the area of Resident Care and Related Services. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Freedom of Movement and 1020 ? Staffing An exit meeting will be conducted to review the inspection findings.
