Comments:

An unannounced monitoring inspection was conducted on 10/25/2021. At the time of entrance 51 residents were in care. The sample size consisted of five resident records, five staff records including a review of criminal background checks for new hires since the last mandated inspections. No violations were found today, and exit interview held with Administrator.
