Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 10/6/2020 and concluded on 10/8/2020. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported the current census was 9. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 2 resident records, 2 staff records, staff schedules, Health Care Oversight, Fire Inspection, Health Department Inspection, Fire and Emergency drills, and Dietitian Oversight submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-320-A
Description: Based on record review and discussion, the facility failed to ensure the physical examination report listed any known allergies and descriptions of the person?s reactions.
Evidence: 1. Resident #2?s physical examination report dated 4/16/2020 identified the resident is allergic to Lisinopril; however, the physical examination did not contain a description of the person?s reactions to the known allergy. 2. Staff #1 acknowledged Resident #2?s physical examination dated 4/16/2020 did not include the aforementioned information.

Plan of Correction: Staff will ensure when the resident's next physical exam is due, the description of the resident's reaction to the known allergy is included by the physician.

Standard #: 22VAC40-73-650-A
Description: Evidence: 1. Resident #2?s September 2020 Medication Administration Record (MAR) showed Resident #2 was not given the prescribed Nicotine Patch 7 mg daily to stop smoking. 2. Resident #2 refused to use the Nicotine Patch 7 mg the entire month of September and there was no documentation that the facility had contacted the physician to discontinue the Nicotine Patch 7mg. 3. Staff #1 acknowledged the facility did not obtain a discontinue order for the Nicotine Patch 7 mg.

Plan of Correction: Staff will contact physician to obtain a discontinue order for Nicotine Patch.
