Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 11/13/2020 and concluded on 11/13/2020. A self-reported incident was received by the department regarding an allegation in the area of personnel. The administrator was contacted by telephone to conduct the investigation. The licensing inspector received the allegation of non-compliance by email and confirmed the information by telephone. The evidence gathered during the inspection supported the allegation of non-compliance with standards or law, and violations were issued.

Description:	Based on documentation and interview, the facility failed to have an administrator of record. EVIDENCE: 1. An email was sent by one of the licensees on 11/2/2020 that stated the administrator's last day was October 30, 2020, and that efforts to hire a new administrator have not worked out. 2. This was confirmed by phone conversation with licensee 1 on 11/9/2020.
