This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 1/27/2021 and concluded on 2/26/2021. A self-reported incident was received by the department regarding allegations in the area of resident care and related services. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation.
The evidence gathered during the inspection supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the monitoring inspection but identified during the course of the inspection can be found on the violation notice.

Standard #: 22VAC40-73-670-3 Description: Based on staff interviews and documentation, the facility failed to have a qualified person to supervise the registered medication aides.
EVIDENCE:
1. On 1/15/2021 the facility had a new administrator. On 1/27/2021 in a phone interview, the new administrator stated there were no nurses working full time at the facility, and the administrator is not licensed to administer medications, nor has the new administrator taken the 68 hour class to be qualified to supervise registered medication aides.
2..On 2/26/2021 in a phone interview, the administrator confirmed there were no nurses working at the facility. In a phone interview with staff 2, the former qualified acting administrator, staff 2 confirmed that she is not the designated assistant administrator, and does not work full time. The record for staff 2 shows the job descriptions of medication aide and facility manager.Plan of Correction: The Administrator, hired January 12, 2021 was made aware that, by State regulations needs to take a 68 hour Registered Medication Aide class.
The Administrator enrolled in the first class available in the area.
The Administrator started the 68 hour medication aid class on February 22, 2021 and will complete the classroom hours as scheduled on March 11, 2021.
Clinical hours will be scheduled in the month of April, 2021.
The 68 hour RMA class should be completed by April 30, 2021.
