Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 5/24/23
The Acknowledgement of Inspection form was signed and left at the facility for the date of the inspection.
Number of residents present at the facility at the beginning of the inspection: 27
The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. The facility was clean and odor free. Two bathrooms had been remodeled. The drain work in the kitchen was almost completed. Previously during cleaning, the drain the snake had been broken off in the system causing a back up and odor. The facility also has approval to replace additional flooring in common areas/hallways and that process has begun. As a part of this monitoring inspection a complaint regarding odor and pets in the building was also addressed and that information can be found in the complaint report. The facility has no pets onsite. They do however, allow visits by therapy animals for the residents which they indicated they really enjoy.

Observations by licensing inspector: Residents and staff were engaged in various activities upon arrival at the facility. Postings were as required and observed to be followed. Outside inspection were current as were any applicable drills. A review of the schedule indicated staffing to meet the needs of the residents. Med review met Board of Nursing guidelines including review of medication administration records (MARs), orders and inspection of the cart and count for schedule II medication. Resident interviewed voiced no issued with services or care in general.
Additional Comments/Discussion:
Fire ? 4/6/23 Fire plan reviewed during fire inspection and evacuation location onsite changed with approval of Fire Marshall.
Health ? 12/31/22
An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
