The LI reviewed the licensed administrator requirements and phase 2 training schedules.

Comments:

A focused inspection was completed on 2/13/2020. The inspection only reviewed the requirements of a qualified administrator, and initial training required.

Violations:

Standard #:	22VAC40-73-150-B-9
Description:	Based on interviews and recordation, it was determined the facility operated under the supervision of an acting administrator pursuant to ?? 54.1-3103.1 and 63.2-1803 of the Code of Virginia more than two times during any two-year period. Evidence: The facility has previously had two acting administrators since 1/1/2019. Staff A, who began as the acting administrator on 1/16/2020, is not licensed in the Commonwealth of Virginia as a long term care administrator.
Plan of Correction:	Licensed Administrator ? Staff B will be in the community Monday through Friday until Staff A is fully licensed. Staff A has submitted her application. DHP has confirmed receipt via email. Staff A will notify DOLP et al when she sits for the test. Staff A's onboarding training ? completed and in the file Staff A?s is scheduled to attend the Phase 2 training on 2/19/2020.
