An unannounced focused monitoring inspection was conducted on 01-16-2020 from 11:30 AM to 2:36 PM. There were 65 residents in care. Resident and staff records were reviewed. Interviews were conducted with staff and residents. Staff statements were reviewed. Incident Reports and Resident Rights were discussed with the Administrator throughout the inspection and during the exit interview. The facility received violations "under" Administration and Administrative Services. The areas of noncompliance were discussed throughout the inspection and during the exit interview. Please complete the -Plan of Correction- for each violation cited on the violation notice and return it to me within 10 calendar days from today, 02-22-2020.

Standard #: 22VAC40-73-70-C Description: Based on record review and interview, the facility failed to submit a written report of each incident to the regional licensing office within seven days from the date of the incident.
Evidence:
1. On 01-04-2020, staff #1 emailed an initial incident report regarding a ?verbal abuse case? involving resident #1 and a staff member. On 01-08-2020, staff #1 emailed documenting that the staff member involved in the incident was terminated. The facility did not submit a written report of the incident to the regional licensing office within seven days from the date of the incident, until requested. The written report was emailed on 01-13-2020.
2. During interview, staff #1 acknowledged the facility did not submit a written report of the incident within seven days from the date of the incident.Plan of Correction: Ed or designee will submit written report of any incident to the regional licensing office within the appropriate amount of time.
