Comments:
Type of inspection: Monitoring
Date of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 7/24/23
The Acknowledgement of Inspection form was signed and left at the facility for on date of the inspection.

Number of residents present at the facility at the beginning of the inspection: 57
The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. The facility was clean and odor free. New flooring had been added and carpeting removed. Postings were as required, and activities and menus followed. Building temps and water temps are logged regularly. Fire doors and secure doors tested on a monthly basis at minimum.
Number of resident records reviewed: 6
Number of staff records reviewed: 5
Number of interviews conducted with residents: 3 plus 1 family member.
Number of interviews conducted with staff: 2
Observations by licensing inspector: Six resident and five staff records were reviewed and found to be in compliance. Interviews with staff, residents and a family member noted complete satisfaction with service with a family member noting staff go beyond what they could have expected in taking care of their family member. Cart review and med observation were in compliance.

Outside inspections and reviews were completed and follow-up as applicable. All drills were as required.
An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. The department's inspection findings are subject to public disclosure.
