Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A unannounced focused inspection was conducted to investigate allegations concerning resident care. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 88. The inspector emailed the administrator a list of items required to complete the inspection. The inspector and licensing administrator reviewed the requested facility documents and record of the resident identified in the report to the Virginia Department of Social Services-Adult Licensing Division. Based on allegations reported regarding the identified resident, the following was reviewed as a part of the investigation: admission and physical examination forms, physician orders, dietary oversight report, meal consumption documentation, monthly weight log, fall risk assessments, incident reports, initial and comprehensive Uniform Assessment Instruments (UAI), Individual Service Plans (ISP), all nurses notes, policy and procedure for missing items, documentation provided by PACE (Innovage), documentation of communication with resident family, hospital visit/discharge information provided and documents provided by the complainant. There was no preponderance of evidence found to substantiate allegations. The information gathered during the inspection determined no violations with applicable standards or law.
