Licensing Inspector (LI) conducted unannounced complaint investigation on 2/25/2020 and 3/16/2020 regarding resident medication administration and resident care. LI reviewed resident record, medication administration records, staff medication aide certification and staff record. Spoke with Executive Director and Director of Nursing. Complaint is deemed valid as a preponderance of evidence gathered during the investigation supported the allegations.
The Violation Notice and complaint closure was delayed due to the pandemic. The Violation Notice regarding the standards deemed valid was sent to the facility on 5/15/2020. Standard #: 22VAC40-73-560-E Complaint related: No Description: Based on interview and documentation review the facility failed to ensure that all resident records shall be kept current.
Evidence: Resident #1 was admitted on 1/28/2020. There are no documented notes in the resident record until 2/14/2020.Plan of Correction: Late entries added for dates available for resident specific communication provided during change of shift communication.
Resident Wellness Director and Executive Operations Office will audit new admission charts within 24 hours after admission to verify timely accurate documentation and comprehensive information.
Standard #: 22VAC40-73-680-C Complaint related: Yes Description: Based on interviews facility failed to ensure that medications shall be administered not earlier than one hour before and not later than one hour after the facility's standard dosing schedule, except those drugs that are ordered for specific times, such as before, after, or with meals.
Evidence: On January 30th and 31st morning medications were administered two hours after the standard dosing schedule. This is outside of the one hour requirement.Plan of Correction: Resident Wellness Director will work with wellness team to ensure team members who administer medications are reeducated to timely administration and documentation. Procedures for medication scanner when administering medications have been updated. Team members will review regularly throughout their shift and at change of shift for any missed medications.
Standard #: 22VAC40-73-680-J Complaint related: Yes Description: Based on interview and documentation facility failed to ensure that in the event of an adverse drug reaction or a medication error, medication administration staff shall document actions taken in the resident's record.
Evidence: On January 30 and 31 a medication error occurred. There is no documentation in the resident record regarding the incident or actions taken.Plan of Correction: Late entry noted for both occurrences, from information communicated during change of shift report for each day.
Resident Wellness Director will work with wellness team to ensure team members who administer medications are reeducated to timely documentation. In the event a medication is missed or administered outside of normal time, physician will be notified, and resident will be assessed to include vital signs. Family will also be notified, and documentation made in chart of occurrence.
