Comments:
An unannounced complaint inspection was conducted on February 13, 2020 from 9:20 a.m. to 1:52 p.m. in response to allegations of resident and staff substance abuse, and resident and staff interpersonal relations. There were 103 residents in care. The following was discussed: documentation of incidents and resident?s notes, staff statements regarding allegations, staff drug testing, staff record requirements, Individualized Service Plan (ISP) accuracy regarding legal representatives, and physical examination/tuberculosis dates of residents. The complaint is not valid.

Please submit your ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it within 10 calendar days. The plan of correction must indicate how the violation will be or has been corrected. The plan of correction must include: 1. Step(s) to correct the noncompliance with the standard(s) 2. Measures to prevent reoccurrence 3. Person(s) responsible for implementing each step and/or monitoring any preventative measures.

Description: Based on record review and interview, the facility failed to ensure health information was maintained at the facility and included in the staff record for the initial tuberculosis examination and report.

Evidence:

1. Staff #1?s date of hire was 08-01-19. There was no documentation of an initial tuberculosis examination and report in the record or in the facility.

2. Staff #2 observed and confirmed staff #1 did not have an initial tuberculosis examination and report on file.

Plan of Correction: 1. As of 2/6/20 and 2/13/20 all staff hired 8/1/20 were re-tested as previous documentation was discovered missing.

2. All charts were audited 3/13/20 to ensure initial Tuberculosis screening was completed upon hire and available in chart.

3. The Administrator or her designee will ascertain all future employees will have screening completed upon hire before starting orientation.
