Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An renewal inspection was initiated with the licensee on 5/3/2021 and concluded on 5/6/2021. The licensee reported a census of 67 residents, The inspector reviewed information submitted for five staff and five residents. medication administration record and physician orders, fire and health inspection documents, Dietician report, health care oversight, staff schedules, etc. for compliance. Information gathered during the inspection determined non-compliance with applicable standards or laws. The non-compliance is noted on the violation notice issued to the facility.

Description:	Based on a review of five resident files on 5/5/2021 during a remote inspection, the facility admitted and retained one individual with a condition prohibited by 22 VAC 40-73-310H. Evidence: The documented move-in date for resident # 1 is 3/10/2021. The admission physical examination for resident # 1 dated 3/4/2021 documented a prohibited condition - "Psychotropic medications without appropriate diagnosis and treatment plans".
