Comments:
An unannounced monitoring inspection was completed on February 25, 2020 from 1:00 p.m. to 6:24 p.m. Upon arrival the licensing representative met with the administrator during the entrance conference. The facility reported the census as 114 residents in care. After the entrance conference, the licensing representative and the administrator completed a building tour. The record review encompassed ten (10) resident records and five (5) staff members. A medication pass was observed as well as general resident activities. One new violation was cited during this monitoring inspection.

Please submit a written plan of correction within the next ten calendar days to address the areas of non-compliance. Your plan should specify how you will correct each violation, the intended plan for compliance, a date of correction for each cited violation, the job title of the identified staff responsible for implementing the corrections, and the monitoring preventive measures. Violations:
Standard #: 22VAC40-73-930-A
Description: VIOLATION: Based upon observation during the building inspection, the facility failed to have a signaling device easily accessible to the resident in his bedroom that would alert staff for the resident's need for assistance.

EVIDENCE: During the building tour, the licensing representative noticed that the call system in bedroom #137 failed to have an activation draw string / handle that would make the emergency call system accessible to the resident.

Plan of Correction: The broken pull cord in room #137 was replaced. A visual inspection of every call bell pull cord was completed with repairs made that date as needed. The monthly call bell checklist was modified to included visual verification of call bell pull cords. The checklist is completed by the Maintenance Director. The monthly call bell inspection will be reviewed at the monthly Safety Committee to determine if any trends can be identified with corrective action taken if needed. The executive director will be responsible for the implementation of this corrective plan.
