Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 08/04/2020 and concluded on 08/05/2020. A self-reported incident was received by the department regarding allegations in the areas of 22VAC40-73-(7) RESIDENT ACCOMODATIONS AND RELATED PROVISIONS. The Administrator and staff person in charge was contacted by telephone to conduct the investigation. The licensing inspector emailed the Administrator and staff person in charge a list of documentation required to complete the investigation.The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Violations:
Standard #: 22VAC40-73-460-D
Description: Based on interviews and record review the facility failed to provide supervision of resident schedules, care, and activities, including attention to specialized needs, such as prevention of falls and wandering from the premises.

Evidence #1: On 08/05/2020 while reviewing resident #1's Uniform Assessment Instrument dated 03/29/2020, documents resident #1 requires mechanical and physical assistance with walking, the Uniform Assesment Instrument also indicates, resident #1 does not perform stairclimbing.

Evidence #2: Resident #1's Individualized Service Plan with the need date identified 03/29/2020 reads, " Resident unable to walk on own. Resident has weakness in legs and is dependent of wheelchair for daily safe mobility. Staff will assist Resident up X2 staff members when resident becomes anxious and insist on standing up and walking short distances to keep resident from getting aggitated".

Evidence #3: While reviewing the facility "PT- Therapist Progress & Updated Plan of Care" dated 06/29/2020 documentation reads, " Impact of Burdenof Care/Discharge Prognosis Patient requires physiical assistance with transfers and mobility."

Evidence #4: Licesning Inspector recieved a self reported incident provided by staff #2, on 05/28/2020 which reads, " On 05/27, at approximately 5:15pm, Resident #1 exited the memory care neighborhood. Within minutes of the alarm door going off, resident was found on the first flight of stairs in stairwell #1. Resident was assessed by LPN charge nurse. Resident had no injuries noted. Resident was assisted back to the memory care dining room."

Evidence #5: On 08/04/2020 while conducting a phone interview with staff #1, staff #1 stated, " Heard the alarm engaged for the memory care unit and went to look. Upon arrival resident #1 was sitting on the 7th step outside of the facility memory care unit." Staff #1 confirmed that resident #1 uses a wheel chair and requires physical assistance with walking and staff also confirmed that resident #1 was unsupervised when located.
