Comments:

The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. An unannounced renewal inspection was conducted on 8/18/2022 (1:30 pm - 4:45 pm ). At the time of the inspection there were 21 residents in care. Three (3) resident and three (3) staff files were reviewed as well as other required documentation. All new personnel records were reviewed since the last inspection for criminal history record reports and all were in compliance. Medication administration records, menus, and all postings and an activity were observed. Building and grounds were inspected. An exit meeting was held with the Administrator and Regional Director of Operations. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website should the facility be issued a license to operate. The department's inspection findings are subject to public disclosure.
