Comments:

This inspection was conducted by Licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. The Administrator was contacted by telephone and email to conduct the inspection. Management has submitted information requested by the Inspector for review of the initial application required to complete the initial inspection. Inspector reviewed initial letter of potential selling of the property, letter sent to families of the change in ownership, the initial application, the Disclosure statement for the facility, the lease agreement, floor plans and some policies and procedures. The information gathered during the inspection determined no violations with applicable standards or law. A Conditional license will be recommended.
