Comments:
A monitoring inspection was initiated on 11/2/2021 and concluded on 11/4/2021. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 63. The inspector emailed the administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 4 resident records, 4 staff records, submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 11/4/2021. An exit interview was conducted with the administrator and licensee on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-100-C-1
Description: Based on observations made during medication cart audits, the facility failed to follow blood glucose monitoring practices consistent with the CDC recommendations.

EVIDENCE:
1. Resident #'s 8, 9, and 10 all require blood glucose monitoring. The individual glucometers for each of these residents were not labeled with the resident name.

Plan of Correction: 1. Medication staff will have in-service on 11/12/21 to establish that not only glucometer that are labeled but the glucometer itself inside case must by labeled as well. Medication staff received verbal reprimand. All individual glucometers labeled 11/5/21. [sic]
