Technical Assistance:

A completed Renewal Application must be submitted prior to the expiration of the current license. The facility should receive an application in the mail, however if an application has not been received one can be obtained from the DSS web site or by calling the main office at (276) 206-0492.

Comments:

An unannounced renewal study was initiated on 1/25/2022 and concluded on 1/26/2022. At the time of entrance eight residents were in care. The sample size consisted of four resident records, and two staff records. Resident and staff records and other documentation were reviewed. Residents were observed eating breakfast and engaging in activities including a spa day. Medication administration was observed. Violation notice issued, risk ratings reviewed and exit interview held. Areas of non-compliance are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. The plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #:	22VAC40-73-260-A
Description:	Based upon a review of records, the facility failed to ensure that each direct care staff member who does not have current certification in first aid as specified in subdivision 1 of this subsection shall receive certification in first aid within 60 days of employment. Evidence: According to the staff roster, Staff #2 was hired on 9/2/2021 as a certified nurses assistant. Certification of completion of first aide training was not found in the file for Staff #2.
Plan of Correction:	Schedule a first aid training through our pharmacy provider. A scheduled training for March 2022 will be moved to a closer date. We will work with our vendor to get the soonest possible date for this training.
