Type of inspection: 60 Day Monitoring Date of inspection and time the licensing inspector was on-site at the facility for the inspection: 7/26/22, 9:45am to 12 noon The Acknowledgement of Inspection form was signed and left at the facility on the date of the inspection. Number of residents at the facility on the date of the inspection: 75 The licensing inspector observed the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 10 Number of staff records reviewed: Background checks for all staff hired since 2/14/2022. Number of interviews conducted with staff: 3 Observations by licensing inspector: medication storage and administration, postings An exit meeting was conducted to review the inspection findings.
