Recommendation to implement coded door system on the secured memory care unit for offices, spa and uncommon areas due to residents having a serious cognitive impairment.

Comments:

An announced initial inspection was conducted on 05/02/2022 in response to a new application for licensure related to a change of licensee. Policies and procedures have been submitted for review and discussed with the licensee to ensure requirements are met per Commonwealth of Virginia State Board of Social Services Standards for Licensed Assisted Living Facilities 22 VAC 40-73. All required inspections are current. The licensee shall complete new sworn disclosure statements and criminal history reports for all staff. A new resident agreement and facility disclosure statement to be provided to all residents and/or legal representatives for review and signature. A monitoring inspection will be completed in sixty days to ensure compliance.
