Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 9/30/2024, 10/1/2024,10:30 a.m.-12:00 p.m. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint/self-reported incident were received by VDSS Division of Licensing on 9/1/2024 a self reported incident and on 9/16/2024 a complaint regarding allegations in the area(s) of: Personnel, Resident Care and Related Services, Resident Accommodations and related Provisions, Building and Grounds. Number of residents present at the facility at the beginning of the inspection: 8 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: NA Number of staff records reviewed: NA Number of interviews conducted with residents: 8 Number of interviews conducted with staff: 2 Observations by licensing inspector: Medication Administration Records, Physician's orders, food supplies, lunch meal, observation of residents hygiene, resident's personal allowance receipts. Additional Comments/Discussion: Staff members were given the opportunity to ask questions. Investigated by 2 Licensing Inspectors. The evidence gathered during the investigation did not support the (allegation(s)/self-report) of non-compliance with standard(s) or law.
