Comments:

Two licensing inspectors conducted a one day unannounced mandated renewal inspection at Showalter Center on 12/5/2019. The inspection started at 10:20 am and concluded at 2:35 pm. During this inspection the mid-day meal was observed, noon medication pass observed, building was observed, medication cart audits were conducted, staff and resident interaction and activities were observed, required postings were checked, and a sample of resident and staff files were reviewed. The facility had 78 residents in care at the time of inspection. An exit meeting was conducted with the administrator and other key staff on 12/5/2019 at that time opportunity was given to find items that were not available in files. As a result of this inspection no violations are being cited. Please review, sign, and return the inspection documents back to the licensing office within 10 calendar days (01/02/2020 ) of receipt.
