Comments:
An unannounced renewal inspection was initiated and completed on 3/6/24 from 10:30am until 12:30pm. At the time of entrance, there were 58 children in care with 12 staff present. A sample of 6 children's records and 6 staff records were reviewed. Children were observed engaging in free play, circle time and eating lunch. Lunch service procedures and diapering/handwashing procedures were also observed. First aid and emergency supplies, documentation of emergency practice drills, medication, children's injury reports and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are listed on the violation notice issued to the center and were discussed with the assistant director during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Repeat Violation
Based on record review and interview, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence:
1. The record for staff 1 (date of employment: 1/23/24) contains a tuberculosis screening dated 2/21/24, which is after the date of employment.
2. Center management confirmed that the tuberculosis screening was completed after staff 1's first date of employment.

Plan of Correction: The center responded with the following: This was an oversight as center management looked at the wrong date documented on staff 1's TB screening. Going forward, management will closely review all required documents submitted by staff to ensure accuracy.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas of the center shall be maintained in a clean and safe condition.

Evidence:
In the four-year old classroom, there were approximately 25 staples lying on the rug under the bulletin board and accessible to the children in care. The licensing inspectors observed children playing in this area of the classroom during the inspection.

Plan of Correction: The center responded with the following: The staples were cleaned up off the floor during the inspection. Staff had taken down the bulletin board the evening before and the staples did not get vacuumed up. Staff will be reminded on center cleaning procedures and to double check to ensure there are no unsafe items within reach of children.
