Technical Assistance:
Discussed subsidy requirements for staff and the requirement for TB test within 30 days prior to employment for subsidy vendors.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 3/09/2021. The director was contacted by telephone to initiate the inspection. There were (80) children present and (12) staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed (4) children?s records and (6) staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of 6 staff records, the center did not have documentation of a tuberculosis test or screening for staff within 21 days of hire or within 12 months prior to employment.
Evidence:
1. Staff #1 was hired on 9/21/2020 and had a TB screening or test dated 3/28/2019.

Plan of Correction: I'll make sure it is within 30 days of hiring .

Standard #: 22VAC40-185-260-B
Description: Based on interview, the center did not have an annual approval from the health department.
Evidence:
1. The last health department inspection was dated 6/08/2019 and Staff A stated they had not been out to do an inspection this past year.

Plan of Correction: I will call them to schedule an inspection.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of 6 staff records, the center did not have the fingerprint results prior to 1st day of employment for a staff member.
Evidence:
1. Staff #1 began at the center on 9/21/2020 and the center did not receive the fingerprint results until 9/28/2020.

Plan of Correction: I will verify that I have received the results before they start.
