This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.
A monitoring inspection was initiated and concluded on 8/17/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 82 children present, ranging in ages from 5 months to 9 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 7 staff records were reviewed.

Standard #: 22VAC40-185-150-B Description: Based on review of 4 children records, the center did not have immunization documentation that was signed or stamped by a physician, his designee, or an official of a local health department.
Evidence:
1. The center had a copy of Child #1's immunization record, but it was not signed or dated by a physician or designee.Plan of Correction: I messaged the parent to have the doctor sign and date the immunization record and send back.
Standard #: 22VAC40-185-160-A Description: Based on review of four staff records, the center did not have documentation of a tuberculosis (TB) test or screening for staff within 21 days of hire or within 12 months prior to hire.
Evidence:
1. Staff #3 had a TB test or screening dated 5/31/2019.Plan of Correction: I asked her to do it today.
