An investigation was initiated on 8/4/23 in response to a self reported incident on that same date regarding supervision. Documentation related to the incident was obtained from the center and reviewed. An on-site inspection was conducted on 10/18/23 and interviews with staff were conducted. Based on information gathered during the investigation, the self reported incident is determined to be not valid. No violations were issued.
