Comments:
An unannounced monitoring inspection was initiated and completed on 02/07/204. The inspector was on site from 1:15pm until 5:10pm.

There were 103 children, ages 3 months to 11 years old under the supervision of 12 staff. Children were observed during indoor free play and lunchtime. Four children's records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1.On the playground, the bottom step to the children?s boat was cracked and broken, creating a gap. This is an entrapment hazard to children.
2. The picnic table under the outdoor play structure on the School age playground, had a board that was broken and splintered. This creates a snagging hazard to children.
3. Children (ages 3-5 years) were observed during the nap period sleeping on mats. The plastic bags that are used to store the mats (after nap) were placed under the mats where the children?s heads were laying. During nap, several children had transitioned from the mat and their heads were observed laying on or near the plastic bags under the mats. To avoid danger of suffocation, plastic bags must be kept away from babies and children.

Plan of Correction: 1. The children's boat will be thrown away.
2. Maintenance will be replacing the board.
3. The PD has spoke with the teachers and the bags are not stored with children. 2/7

Description: Based on review and interview, the center failed to ensure that the record of each child?s feeding schedules.

Evidence:
In the infant 2 class, there were no child feeding schedules available to staff.

Plan of Correction: The PD has spoken with the infant 2 teachers on feeding schedules and has been fixed.
