An unannounced monitoring inspection was conducted on 02/07/2024 and concluded with an electronic record review on 02/14/2024. There were 27 children, ages 4 years - 11 years, and 4 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during arrival, and self-directed activities. A total of 5 children?s records, and 4 staff records were reviewed. The inspector discussed the following with the provider: medication and children?s records. The inspector arrived for the inspection at approximately 6:45 am and departed at approximately 8:10 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-B-4 Description: Based on record review and staff interviews, the center failed to obtain a copy of a child abuse and neglect search from any state in which the individual has resided in the preceding five years.
Evidence: Staff 3 has resided outside of the state of Virginia within the preceding five years. There was not an out of state child abuse and neglect registry check for Staff 3 from all states in which the individual has resided in the preceding five years available for review during the inspection. Provider submitted request on 09/01/2023 without a follow-up for results. The documented hire date in the Record for Staff 3 was 09/25/2023. Staff 1 and Staff 5 confirm this information was not available for review.Plan of Correction: Provider plans to resubmit request.
