A Subsidy Health and Safety inspection was conducted on Thursday, February 27, 2020 to review supplemental health and safety requirements. The licensing inspector was on site from approximately 10:00 AM until 3:00 PM. The following ratios were observed with a total of 49 children present during the inspection:
Infants - 9 children with 3 staff
Toddlers - 6 children with 2 staff
Twos - 17 children with 3 staff
Preschool - 9 children with 2 staff
Pre-K - 8 children with 1 staff
School Age were not present during the inspection.
The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, playing with toys, listening to music, looking at books, learning about dinosaurs, playing a memory game, playing in the sensory table, having their diapers changed, taking bathroom breaks and washing hands, eating lunch and resting at nap time. All areas of the center including classrooms, hallways, kitchen, bathrooms, vehicles and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children. During the inspection, six children's and six staff records were reviewed.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11 Description: Based on documentation review, the vendor did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
The record for Child #2 indicated the child had a food allergy. The record did not contain a written allergy care plan. A member of management confirmed this information during the inspection.Plan of Correction: We will have the parent submit an allergic action plan with the required information.
Standard #: 22VAC40-665-540-A Description: Based on documentation review and inteview, the vendor did not ensure two staff members had been issued a statement that staff were determined to be free of communicable tuberculosis (TB). The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
The record for Staff #2 contained a TB screening dated 02/21/2020. The documented hire date for Staff #2 was 02/19/2020. The record for Staff #5 contained a TB screening dated 12/11/2019. The documented hire date for Staff #5 was 12/09/2019. A member of management stated she thought the TB test was required to be obtained before staff came in contact with children.Plan of Correction: We will make sure all future staff do not begin employment before obtaining a TB screening that was completed within the last 30 days.
Standard #: 22VAC40-665-780-A-3 Description: Based on documentation review and interview, the center did not ensure a lockdown drill was practiced at least annually.
Evidence:
The emergency drill log indicated documented the last practiced lockdown drill occurred on 11/01/18. A member of management stated shelter-in-place drills have been practiced. The emergency drill log indicated the last practiced dates of the shelter-in-place drills occurred on 2/25/19 and 3/29/19.Plan of Correction: We will documented the emergency drills practiced in the future as lockdown drills.
