Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 1/7/2025. The on-site inspection began at 10:25am and ended at 11:45am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 2 children present and 1 caregiver; the point total was 6. The inspector reviewed 3 children?s records and 2 caregiver/household records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-B
Description: Children's records are required to contain parental home and employment addresses and phone numbers. The record for child #1 was missing both parents work phone numbers. The record for Child #2 was missing the parent employment address and phone number. Both children were present during the inspection.

Plan of Correction: The provider will contact the parents to receive the missing employment information.
