Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site renewal inspection was initiated and completed on October 4, 2024, as a part of the licensure period. The on-site inspection began at 10:45am and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 29 children present and six staff. The inspector reviewed five children?s records and five staff records on-site.
This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 24, 2024. A POC submitted after this date will not appear on the public website.

Description: REPEAT VIOLATION

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The record for Child #1 (20 months old) contained documentation of immunizations from 2023. Child #1 has been enrolled in the center for 17 months.

Standard #: 8VAC20-780-160-A
Description: REPEAT VIOLATION

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The record for Staff #1 contained documentation of a TB screening obtained three days after employment. The record for Staff #2 contained documentation of TB screening obtained two months prior to beginning employment.

Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #3, enrolled for four months, was missing the address of two designated people to call in an emergency if a parent cannot be reached.

Description: Staff records shall be kept for each staff person. The record for Staff #4, employed for 17 months, did not contain the name, address, and telephone number of a person to be notified in an emergency and information about any health problems that may interfere with fulfilling the job responsibilities. This information must be kept at the center.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Four containers of disinfectant and cleaning products were located in an unlocked cabinet underneath the diaper changing table in the Infant Classroom. The containers had warning labels, ?Keep out of reach of children,? . Infants were observed playing on the floor near the changing table.

Standard #: 8VAC20-780-500-B
Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. A soiled pull-up was observed next to a trash can on the bathroom floor located between the Early Preschool and Preschool classroom.

Description: Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. A prescription medication for Child #4 was not labeled with the child?s name, dosage amount and times to be given.
