A monitoring inspection was conducted from 10:30 AM to 12:30 PM in conjunction with a subsidy health and safety inspection. At the time of the inspection, there were 69 school age children in care with ten staff. At the time of the inspection, children were observed participating in free choice activities, playing group games in the gym and preparing for lunch. The sample size was five staff and five children's records. Medication was also reviewed. Violations are noted on the attached violation notice.

Standard #: 8VAC20-780-160-A Description: Based on review of five staff records, it was determined that the facility did not ensure that each staff member had a negative tuberculosis (Tb) screening on file prior to working with children.
Evidence:
1. Staff #3 (date of hire 5/18/2023) had a Tb screening on file dated 5/29/2023 - the day after they were employed.
2. Staff #5 (date of hire /2026/173) had a Tb screening on file dated 6/18/2023 - the day after they were employed.Plan of Correction: Facility administrator stated that all staff are now obtaining Tb screenings prior to employment.
Standard #: 8VAC20-780-60-A-8 Description: Based on a review of five children's records, it was determined that the facility did no ensure that that was a written care plan for children with food allergies.
Evidence:
1. The record for child #1 (date of enrollment 4/10/23) indicated that the child had allergies, but there was no allergy plan on file.
2. Staff #6 verified that there was no allergy plan on file.Plan of Correction: Facility director contacted the parent during the inspection to obtain the written allergy plan.
Standard #: 8VAC20-780-270-A Description: Based on review of the physical plant, inside and outside, it was determined that the facility did not ensure that all equipment was safe.
Evidence:
1. There was a six inch crack in the red twisty slide that is a part of the composite piece of playground equipment on the school age playground.
2. Staff #6 verified that there was a crack in the slide.Plan of Correction: Facility administrator said that the school age children will not use the playground equipment until it is considered safe. Instead, they will use the open fields available to them
Standard #: 8VAC20-780-560-G Description: Based on inspection of the lunch boxes, it was determined that facility did not ensure that all lunch boxes with food brought from home is labeled with the owners name and the date.
Evidence:
There were at least five lunch boxes in the school age area that, although labeled with the owners name, were not dated.Plan of Correction: Staff will be retrained to ensure that as students arrive their lunch boxes are labeled with the owner's name and the date.
