Comments:
An unannounced monitoring inspection was conducted on-site June 20, 2024 from 12:30pm until 1:40pm. The director was available during the inspection. There were 79 school-age children present with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-440-A
Description: Based on observation and interview, the center failed to ensure cots or rest mats shall be provided for children during the designated rest period and not occupied by more than one child at a time.
Evidence:1. In room 19, children were laying down to rest. There were no mats or cots. Three children were laying on the bare floor.
2. In room 20, children were being asked to rest for nap. There were no mats or cots for the children to lay on. Children were laying on towels on the floor.
3. Staff confirmed there are no mats or cots at the center.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview, the center failed to ensure medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. In rooms 17, 18, 19, 20 and 21, there were unlocked medications. Medications are being kept in unlocked bags in each room. There were approximately 13 unlocked medications total. Staff in each room confirmed medication is kept in an unlocked bag.

Description: Based on observation and interview, the center failed to ensure a first aid kit shall be on each floor of each building used by children.
Evidence: There is not a complete first aid kit on the 2nd floor where children are in care. Staff confirmed there is not a complete first aid kit on the 2nd floor.
