An unannounced monitoring inspection was conducted on September 26, 2023 with the center director. There were 39 children present, ranging in ages from 5 months to 4 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: Based on record review and interview, the center failed to ensure that all employees shall undergo background checks every five years.
Evidence:
1. The staff record for Staff #3 contained a national criminal background check dated 7/9/2018. Staff #3should have undergone a criminal background check on or before 7/9/2018.
2. The director verified that a criminal background check had not been completed for staff #3 since 2018.Plan of Correction: Will get fingerprints done immediately.
