This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
The complaint inspection was initiated on October 12, 2022 and concluded on February 14, 2022. The local agency representative and inspector conducted a joint investigation regarding concerns related to staffing and supervision received by the department on October 1, 2021. Interview with staff and review of documentation was completed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. There were violations cited and please review the violation notice for more details. Standard #: 8VAC20-780-340-F Complaint related: No Description: Based on interview and review of documentation, the center did not ensure all children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence: On October 12, 2021, interview with Staff#1 confirmed the Child#1 left the school without adult supervision and was found wandering outside of school parking lot when the parent arrived for pick up on October 1, 2021.
