An unannounced renewal inspection was completed on Thursday, April 20, 2023 at the program. There were 30 children present with 5 staff present supervising including an administrative/advocate staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22.1-289.035-B-4 Description: Based on review of staff records and interview, in 1 of 5 records the center did not ensure to obtain a copy of the results of a criminal history record and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: In Staff#5's record (date of hire: 0912/22) there were no search results for 3 states (CO, AK, TX) listed found in the record.
