An unannounced monitoring inspection was conducted today between the hours of 9:30am-11:00am. There were a total of 59 children with 11 staff members present. Children were observed during diapering, hand washing, circle time and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-140-A Description: Based on documentation review, each child did not have a physical examination within one month after attendance.
Evidence:
1. Child #2's record (Start date-02/28/2022) did not have documentation of a physical examination.Plan of Correction: The parents were emailed to notify of missing physical examination. The parents will get a copy of the physical examination and the center will document the record.
Standard #: 8VAC20-780-290-A-3 Description: Based on observation, two electrical outlets did not have protective covers.
Evidence:
1. In the Toddler's classroom, two electrical outlets did not have protective covers.Plan of Correction: Protective covers were put on the electrical outlets during the time of the inspection.
Standard #: 8VAC20-780-520-C Description: Based on observation, diaper cream was not labeled with child's name and was accessible to children.
Evidence:
1. In the bathroom on a low shelf in the Two year old classroom, two containers of diaper cream were not labeled with the children's names and was accessible to children.Plan of Correction: The diaper creams were moved to a higher shelf and labeled with the children's names during the time of the inspection.
