An unannounced monitoring inspection was conducted on April 30, 2024 between the hours of 9:30am-11:30am. There were a total of 62 children with nine supervising staff members present. Children were observed during circle time, reading books, engaged in group activities and outdoor play time. A selection of staff and children records and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22.1-289.035-A Description: Based on record review and interview, the center did not obtain repeat background checks for an employee every five years.
Evidence:
1. Staff # 6's record (Date of hire-06/02/2016) did not contain a repeat criminal background check every five years. Staff #6's criminal background check expired on 08/15/2023.
2. Staff #6 confirmed that they had not completed a new criminal background check.Plan of Correction: Staff #1 background appointment is scheduled for Thursday, May 2, 2024. Moving forward, we will quarterly check employee files for updated documents needed.
Standard #: 8VAC20-780-160-A Description: Based on record review, each staff member did not submit documentation of a negative tuberculosis(TB) screening at the time of employment.
Evidence:
1. Staff #1's record (Date of hire-01/22/2024) did not contain documentation of a negative Tb screening.Plan of Correction: Staff #2 has pending TB results. Moving forward, we will ensure employee licensing documents are presented before the hiring date.
Standard #: 8VAC20-780-60-A Description: Based on record review, children's records did not contain the required information.
Evidence:
1. Child #3's record (Start date-10/18/2021) did not contain documentation of child updates and confirmation of up to date information in the child's record.

Child #5's record (Start date-11/29/2021) did not contain documentation of child updates and confirmation of up to date information in the child's record.Plan of Correction: Children's records were updated on 05/02/2024. Moving forward, we will ensure children's files will be updated yearly or as parent's information has changed.

Staff #1's record (Date of hire-01/22/2024) did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, orientation and training.Plan of Correction: Proof of education was submitted by staff on 04/30/2024. Moving forward, we will ensure licensing documents are presented before the hiring date.
Standard #: 8VAC20-780-510-I Description: Based on documentation review, the center did not obtain written authorization from a parent and physician for a prescription medication.
Evidence:
1. Child #6 has prescription diaper cream on site. Child #6's record does not documentation written authorization from a parent or physician for administration.Plan of Correction: The medication was returned to the family. Moving forward, we will ensure to have proper medication authorization forms on hand for families the moment that it is presented.
Standard #: 8VAC20-780-520-C Description: Based on observation, diaper cream was not labeled with the child's name.
Evidence:
1. A diaper cream for Child #7 was not labeled with their name.Plan of Correction: Expired diaper cream was returned to the family. Moving forward, we will ensure OTC meds are labeled from the parent during drop off and sent home when expired.
