Comments:
The licensing inspector conducted an unannounced inspection on Thursday, June 6, 2024 in response to a self-reported incident that the Department received on May 21, 2024 regarding inadequate supervision. The inspector was on site at the center on Thursday, June 6, 2024 from 10:24 am to 2:01 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The inspector completed an additional interview on June 10, 2024.

The preponderance of the evidence gathered during the investigation validates the self-reported incident of inadequate supervision. Violations were cited regarding supervision and center policies. See the violation notice for more details.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on staff interviews and record review, licensee failed to ensure operational responsibilities that the center?s activities and services are maintained in compliance with the center?s own policies and procedures that are required by the standards.

Evidence: Staff members (S1, S2, and S3) acknowledged in interviews that on 5/20/24, a child (C1-age 5) was left out on the playground alone when the rest of his class went inside. The center?s policy for playground safety states that staff are to follow the Face to Name process as each child crosses the threshold into the classroom and to complete a final Face to Name once they are all inside the classroom and the door is closed. S3 acknowledged that they failed to do a Face to Name headcount upon returning to classroom as their policy requires.

Plan of Correction: Event was self-reported by Director to Virginia Department of Education to maintain transparency and to keep safety a number one priority in our school. Employees were re-trained in Face to Name Procedures the following day. Emphasis was reiterated on doing the procedure as children cross the threshold of the door. A Face to Name was done outside, but the 5-year-old child ran and hid behind the playset. When the teachers entered the room, and the mother arrived, it was determined he was still outside for approximately 2 minutes. Disciplinary action was taken immediately regarding the failure to follow company procedures regarding the Face to Name while the child is crossing the threshold. The director and assistant director audit Face to Name reports throughout the day to ensure compliance.

Description: Based on staff interviews and record review, the center staff failed to always ensure the care, protection, and guidance of a child when they were supervising him.

Evidence: Staff members (S1, S2, and S3) acknowledged in interviews that on 5/20/24, a child (C1-age 5) was left out on the playground alone when the rest of his class went inside. Staff were not aware that the child was left outside until the child?s mother arrived for pick up and C1 could not be located. Staff found C1 on the playground. C1 was alone on the playground for approximately two minutes.

Evidence: Staff members (S1, S2, and S3) acknowledged in interviews that on 5/20/24, a child (C1-age 5) was left out on the playground alone when the rest of his class went inside. C1 was out of sight and sound supervision of staff for approximately two minutes.
