An unannounced non-mandated inspection was initiated on 11/10/2022 and concluded on 12/14/2022. The purpose of the inspection was to investigate a self-report alleged incident that occurred at the center 11/08/2021 regarding supervision. Three staff were interviewed, one child's record, two staff records, the employee handbook and sign in and sign out documents were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Description: Based on interviews, the center failed to ensure the care and protection of a child at all times.
Evidence:
1. On 11/8/21 Child #1, (age 2), was left on the playground unsupervised for approximately two minutes.
2. Staff #3 stated in an interview on 12/6/2021, when she went out to the playground that Child #1 was screaming for his mom and dad. She yelled for Child #1 to come to her and she gave him a hug.
3. Staff #1 and Staff #2, stated that when Child #1 was returned to them by Staff #3 crying and upset. They gave him a hug and he stopped.Plan of Correction: The lead teacher in the classroom received disciplinary action. The director met with staff and let them know if the situation occurs again administration should be notified the same day.
Standard #: 8VAC20-780-340-F Description: Based on interviews, the center failed to ensure sight and sound supervision for children under the age of 10 at all times.
Evidence:
1.On 11/8/21 Child #1, (age 2), was left on the playground unsupervised for approximately two minutes.
2.Staff #3 stated in an interview on 12/6/2021, that she was prepping the changing table in her classroom, and looked out on the window and saw and heard Child #1 alone on the playground. Staff #3 went out to the playground and retrieved Child #1 and returned him to his classroom.
3.Staff #1 and Staff #2 verified in interviews that Child #1 was left unsupervised for approximately two minutes.Plan of Correction: A staff meeting was held on 03/28/2022; discussed making sure you know how many children you have at all times, it is important to count the children. Check to make sure the children are signed in and signed out. Count during transitions when you leave, and when you arrive. Reviewed expectations of staff when on the playground. Spread out to ensure all areas of the playground are being supervised, be involved in the children's play, and to count every 15 minutes. New administration will be coming on board in April 2022 will discuss what further trainings may be needed.
