An unannounced monitoring inspection was conducted on 8/31/2023 from 8:45 am-11:00 am. There were 93 children supervised by 24 direct care staff. The physical plant, 5 staff records, 10 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play and playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 8VAC20-780-70 Description: Based on record review and interview, 2 of 5 staff records are missing documentation that two or more references as to character and reputation as well as competency were checked before employment
Evidence:
1. Staff #2 (DOH: 8/8/2017) is missing 1 out of 2 references.
2. Staff #3 (DOH: 11/27/2017) is missing 1 out of 2 references.Plan of Correction: I will ask staff to provide an additional person to send a reference.
Standard #: 8VAC20-780-510-G Description: Based on medication review and interview, 1 of 4 medications was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence: Child #11 has emergency medication that is missing a prescription medication label that includes the child's name, the name of the medication, the dosage amount, and the time or times to be given.Plan of Correction: Talk to parents about obtaining label and box for current Tevapen. Will seek agreement of new pen with a box if the pharmacy can't provide for a current pen.
