An unannounced monitoring inspection was conducted on 11/27/2023. There were 3 children, age 5 months to 4 ? years, totaling 9 points, 1 caregiver and 1 household member present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records , 1 caregiver?s records and 1 household member?s records were reviewed. The children were observed during self-directed play. The inspector arrived for the inspection at 9:50am and departed at 10:48am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-110-B Description: Based on the review of two children?s records, the provider failed to ensure that each report shall be signed by a physician, the physician?s designee, or an official of the local health department.
Evidence:
1. Child #2 start date was 10/05/2023 and on the day of the inspection did not have a physical record signed by a physician.
2. Child #2 start date was 10/05/2023 and on the day of the inspection did not have an immunization record signed by a physician.Plan of Correction: Provider will obtain a signed physical and immunization record from parent.
