Comments:
An unannounced monitoring inspection was initiated on 8/19/2024 and completed on 9/10/2024 in response to a self-report received by the licensing office on 8/15/2024 relating to hazardous substances. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the 9/19/2024.

Violations:
Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Child 1(age 10 months) was able to access a mop bucket left on the floor with cleaning solution. The child knocked over the bucket, spilling the contents on the floor around the child and getting the child?s clothes.

Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care, protection and guidance. On 8/14/2024, 2 staff were aware of a mop bucket with cleaning solution being brought into the infant room for after lunch cleaning. Both staff observed Child 1 (age 10 months), crawling towards the area where the mop bucket was left on the floor. Child 1 was able to access the bucket and knock it over. The contents of the bucket spilled on the floor and around the child, causing their clothes to become wet. The cleaning solution used in the bucket had a warning label to keep out of reach of children and avoid contact with eyes.
