Comments:
An unannounced, on-site renewal inspection was initiated on 9/30/2024 and completed on 9/30/2024, as a part of the licensure period. The on-site inspection began at 10:57AM and ended at 1:22PM. The inspector reviewed compliance in the areas listed above. There were 60 children present and 14 staff. The inspector reviewed 5 children?s records on-site 9/30/2024 and 3 staff records on-site 9/30/2024. This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #4, who had been attending for a year, did not have documentation of an immunization record on file.

Plan of Correction: Provider - I have obtained the state med form; the physical and immunization record for the child's file.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, who had been enrolled for one year.

Plan of Correction: Provider - I have obtained the state med form; the physical and immunization record for the child's file.

Standard #: 8VAC20-780-160-A
Description: Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and be signed by a physician, physicians designee, or an official of the local health department. Staff #3, employed almost a month, did not have documentation of a negative tuberculosis screening signed by a physician.

Plan of Correction: Provider-I requested the employee to have her physician sign the completed result

Standard #: 8VAC20-780-70
Description: The following staff records shall be kept for each person. Staff #2, employed for 2 months, did not contain information, to be kept at the center, about any health problems that may interfere with fulfilling the job responsibilities. Staff #3, employed for almost a month, did not contain information, to be kept at the center, about any health problems that may interfere with fulfilling the job responsibilities.

Plan of Correction: Provider-I have had the whole staff sign the health questionnaire for best practice and have added it to our new hire forms for future reference.

Standard #: 8VAC20-780-500-B
Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that the staff member?s hand touches an exterior surface of the storage system during disposal. The foot operated trashcan for the infant classroom was broken. Staff confirmed that the trashcan was opened by hand after diaper changes.

Plan of Correction: Provider-We have a new diapering trashcan with a working lid to replace the broken one.
